Insurance Form

As an exhibitor, you are required to carry commercial general liability insurance at limits of at least $1,000,000 per occurrence, $2,000,000 aggregate.  The coverage must be evidenced by a Certificate of Insurance with a 10 day cancellation notice.  Policy dates from 2/3/2011 – 2/8/2011. The Certificate must name 2011 V-Twin Expo, Duke Energy Center, Cincinnati, OH & Trade Show Management: Reaction Management, Inc. and Paisano Publication, LLC as additional insured’s.

The V-Twin Expo MUST have your Certificate of Insurance no later than January 14th, 2011.

You have 2 options:

  1. Fax this form to your own insurance company for coverage
  2. Fill out and fax the following information sheet (see downloadable form below) to Shahinian Insurance Services for coverage.
